How do ROCK Inhibitors work differently? 
Within the trabecular meshwork, Rho kinase enzymes promote actin-myosin contraction and increase actin stress fibers and focal adhesions, increasing stiffness and causing resistance to aqueous flow.7ROCK inhibitors work at the cellular level within the trabecular outflow pathway to relax actin-myosin. This causes relaxation within the trabecular meshwork and the inner wall of Schlemm's canal, and increases vasodilation within the episcleral venous system.8 Thus, Rhopressa® decreases IOP by improving aqueous humor outflow through the trabecular meshwork1,7 and decreasing episcleral venous pressure (EVP).3

Rocklatan® brings together the first-in-class rho kinase inhibitor, netarsudil, and market-leading prostaglandin, latanoprost, to help lower IOP with one drop.
Netarsudil is a rho kinase (ROCK) inhibitor specifically designed to target the trabecular meshwork. It is believed to reduce IOP by increasing the outflow of aqueous humor.
 
Latanoprost is a prostaglandin that is believed to reduce IOP by increasing the outflow of aqueous humor through the uveoscleral pathway. Rocklatan® is believed to utilize the mechanisms of action of Rhopressa® and latanoprost.

AR-15512, formerly AVX-012, was acquired by Aerie in November 2019 in the acquisition of Avizorex Pharma, S.L., a Spanish ophthalmic pharmaceutical company developing therapeutics for the treatment of dry eye disease. The active ingredient in AR-15512 is a potent and selective agonist of the TRPM8 cold thermoreceptor ion channel that regulates tear production and blink rate. By stimulating these processes, TRPM8 agonists have the potential to restore tear film volume and reduce ocular discomfort in patients with dry eye. Avizorex completed a Phase 2a study in dry eye subjects in 2019 where positive results support the therapeutic potential to treat signs and symptoms of dry eye.
 
The COMET-1 trial is a randomized, double-masked, vehicle-controlled trial evaluating the efficacy and safety of AR-15512 (TRPM8 agonist) in patients with dry eye disease. Approximately 360 patients in total are expected to be enrolled. Patients will receive either AR-15512 0.0014%, AR-15512 0.003% or AR-15512 vehicle dosed as one drop twice daily in each eye over three months. The primary efficacy endpoints of the clinical trial are ocular discomfort (symptom) and tear production (sign). Patients will be evaluated on multiple efficacy assessments at days 14, 28 and 84; safety will be assessed at all visits. Topline results from COMET-1 are expected in the third quarter of 2021. Aerie is currently exploring sustained-release drug delivery technology to treat diabetic macular edema (DME). Our product candidate AR-1105 is an intravitreal, fully biodegradable PRINT®-manufactured implant that releases the steroid dexamethasone. Our expectation is that this implant will provide a six-month duration of action, thereby reducing treatment burden relative to other treatments for DME.1
 
We filed an Investigational New Drug (IND) application for AR-1105 (Dexamethasone Intravitreal Implant) in December 2018. The FDA has reviewed the IND and it is now in effect. A Phase 2 study in the treatment of macular edema due to retinal vein occlusion (RVO) was initiated in March 2019.
