In heart disease, excessive PDE1 activity may limit the beneficial effects of cAMP or cGMP, so inhibitors like ITI-214 have the potential to act as a therapy. ITI-214 is an inodilator that can improve cardiac function by both increasing the force of heart contractions and reducing the resistance to pushing blood through the vascular system.&nbsp;Preclinical research has shown that ITI-214 increases cardiac contractility and decreases vascular resistance without increasing abnormal heart rhythms, and the clinical results announced herein indicate that similar, potentially therapeutic physiologic effects can be attained in heart failure patients.&nbsp;ITI-214 is being developed for the potential treatment of heart failure with reduced ejection fraction (HFrEF).
Inhibitors of PDE1 block the breakdown of cyclic nucleotides (cAMP, cGMP), potentiating downstream intracellular signaling. The PDE1 enzyme has been shown in vitro to be highly active in a variety of disease states. We are investigating our PDE1 inhibitors to restore normal function in these disease states. PDE1 inhibitors appear to have minimal effect on normal cell function, only acting when cells are stimulated in vitro.
ITI-214 is a potent and selective PDE1 inhibitor and the clinical lead compound in the Company’s PDE1 portfolio. ITI-214 is being investigated to treat a variety of neurological, cardiovascular and immune system diseases. It is currently in development for the treatment of symptoms associated with Parkinson’s disease and for the treatment of heart failure.

Pharmacodynamic studies have shown lumateperone acts as a potent serotonin 5-HT2A&nbsp;receptor antagonist with high binding affinity, as a postsynaptic D2 receptor antagonist with moderate binding affinity, as an inhibitor of the serotonin&nbsp;reuptake transporter (SERT) with moderate binding affinity. Lumateperone also has moderate binding affinity to the D1 receptor (which may contribute to the indirect activation of NMDA and AMPA neurotransmission).&nbsp; These drug targets are believed to play an important role in in schizophrenia, bipolar disorder, depressive disorders and other neuropsychiatric disorders.&nbsp; In vitro studies have shown lumateperone has a ~60-fold greater affinity for 5-HT2A&nbsp;receptors&nbsp;vs D2&nbsp;receptors.
